(Registration and Addmission Forms would be filled and Submitted by office Employee Based on Information Provided by Student's at the Institute Management Software).

1. Student's Name : (based on student's Matriculation certificate).

2. Father/Husband/Gaurdian's Name : (based on student's Matriculation certificate).

3. Mother's Name : (based on student's Matriculation certificate).

4. Date of Birth : (based on student's Matriculation certificate).

5. Gender : (Male/Female/Transgender).

6. ID Card : (Anyone in These - Aadhar Card/Votar ID/Pancard/Passport/Driving License).

7. Religion : (Hindu/Muslim/Sikkh/Christian/Other).

8. Social Category : (based on certificate Issued by Authority).

9. Sub Category : (based on certificate Issued by Authority).

10. Student's Mobile No : (to be verify by Institute During Addmission).

11. Gaurdian's Mobile No : (to be verify by Institute During Addmission).

12. Student's WhatsApp No : (to be verify by Institute During Addmission).

13. Student's E-mail ID : (to be verify by Institute During Addmission).

14. Permanent Address : (House/Flat no, village/mohalla, Post Office, Police Station, District, State, Pincode).

15. Correspondence Address : (House/Flat no, village/mohalla, Post Office, Police Station, District, State, Pincode)

16. Student's Educational Details : (based on student's certificate).

17. Exams Name : (based on certificate).

18. Board/Univercity Name : (based on certificate).

19. School/Collage/Institute Name : (based on certificate).

20. Passing Year : (based on certificate).

21. Obtained Marks/Total Marks/Percentage : (based on certificate).

22. Visible Marks on Body : (Compulsory to be Some Courses).

23. Student's Signature : (Compulsory to be all Courses).

24. Student's Left Hand Thumb Marks : (Compulsory to be All Courses).

(At the time of Registration the following documents to be required in Original/Copy Format).

1. Identity Card. :

2. Educational Certificate .

3. Passport Size Colour Photo.

4. Required Fees (All Fees would be submitt in Installment as Ratio of 40%, 40% and 20%).

5. Aadhar Card.. (Compulsory would be Some Courses).

(All Due Fees would be clear by Student before Submittion of Exams Application).
